2008 Denver Kickers Soccer Forms

Program Location is 50th and McIntyre in Golden, CO. Directions can be obtained here.

Registration Instructions:

  1. Download, print and complete the appropriate program registration form.
  2. Download, print and complete the Medical Release Form.
  3. Return all completed forms to the Denver Kickers Youth Registrar.

  4. Be sure to include payment for the selected program.

Uniform Instructions

  1. Indicate the size of the uniform on the registration form.
    • If necessary, contact Theresa Reed at 303-940-7649 for available sizes.
  2. Kickers Kids in stock uniforms will be reserved on a first-come-first-serve basis.
    • Once the inventory is depleted, we will order additional uniforms before August 10.
  3. Include uniform payment only if before above date or confirmed sizes are available.
